Scaling and root planing, explained properly

How it differs from a regular cleaning

A routine cleaning works on the crown of the tooth and just under the gum margin. A deep cleaning goes further down — along the root surface inside the pocket, where hardened deposits sit against the root and keep the inflammation going.

Root planing then smooths that root surface, because a rough root holds bacteria and a smooth one lets the gum tissue reattach to it. That reattachment is the actual goal. Removing the deposit is only half of it.

Because it works below the gumline, you are numbed for it, and it is usually done a quadrant or a half-mouth at a time rather than all at once.

On being told you need one when you feel fine

This is one of the most commonly over-recommended procedures in dentistry, and patients are right to be wary. So here is the standard we hold ourselves to: we will show you the actual numbers, tooth by tooth, and the x-rays showing whether bone has been lost.

Pockets of four millimeters and above with bleeding and visible deposit is a genuine indication. Three millimeters everywhere with a bit of staining is not — that is a regular cleaning, and we will book you a regular cleaning.

If you have been quoted this elsewhere and it did not sit right, bring the treatment plan in. We will look at it with you honestly, including when we think they were right.

The laser, and why we use it here

We pass a diode laser through the pockets alongside the scaling. It targets bacteria in the soft tissue lining of the pocket — the part instruments cannot physically reach — and it seals small vessels as it goes.

In practice patients notice two things: less bleeding during the appointment, and less soreness that evening. It adds a few minutes and no discomfort.

It is not a substitute for the mechanical cleaning. Anyone offering laser treatment instead of scaling is overselling it.

Here’s how the visit actually goes.

    Numbing

    Given time to work properly before we start. This should not be uncomfortable, and if it is, say so and we stop.

  2. 02

    Quadrant by quadrant

    Usually two visits, half the mouth each. That keeps appointments manageable and leaves you able to eat normally on one side.

  3. 03

    Laser pass

    A few minutes per quadrant, painless, alongside the scaling.

  4. 04

    Re-measure at six weeks

    We chart again to see what actually changed. If pockets have not improved, we would rather know and adjust than assume.

  5. 05

    Onto maintenance

    Every three or four months from then on, which is what keeps the result.

Questions

Answered before you ask.

Does it hurt?

Not during — you are properly numbed. Afterwards, expect a few days of tenderness. Patients who have had one elsewhere and found it rough are usually surprised at the difference the laser and unhurried numbing make.

Do I really need it, or is this an upsell?

A fair question and one we would rather you ask. Ask to see your pocket chart and your x-rays. Four millimeters and above, with bleeding and visible deposit, is real. If someone cannot show you numbers, be skeptical.

Will my gums grow back?

The attachment can tighten and pockets can reduce, often substantially. Gum tissue that has already receded does not regrow on its own, and bone that has been lost does not return. Treatment stops further loss.

How many appointments?

Usually two, occasionally four for more advanced cases. Then a re-measure at six weeks and maintenance visits thereafter.
